29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that is greater than 4 feet (1.2 m) above a lower level by covers or guardrail systems: (a) UTILITY DEPARTMENT, COIL MARRIAGE - Employees performed work near the utility/coil marriage area that included tending and cleaning the coil baskets and travelling through the area to empty a waste lubrication tank, and the employer did not ensure that employees were protected from falling into the approximate eight foot deep pit that houses a portion of the coil marriage equipment, on or about September 6, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) UTILITY DEPARTMENT, COIL MARRIAGE - Employees cleaned and labeled coil baskets, and the employer did not guard the rotating conveyor carousels from contact, which thereby exposed employees who walked and worked in close proximity to the rotating carousels to pinch point hazards, on or about September 6, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. Cambridge Lee Industries LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1091880, citation number 1, item number 2 and was affirmed as a final order on March 9, 2016, with respect to a workplace located at 86 Tube Drive, Reading, PA 19605.
