29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a) Cambridge-Lee Industries, LLC., 70 Tube Drive, Reading, PA 19605, Plant 2 Casting Floor, South Mold #1 - On or about September 8, 2023, the caster operators were exposed to burn hazards when they were splashed with the molten copper. The employer provided cotton flame-resistant uniforms that the employees were wearing were not effective, resulting in burn injuries to the employees. The employees were not wearing Kevlar personal protective equipment as prescribed by the personal protective equipment hazard assessment. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace: a) Cambridge-Lee Industries, LLC., 70 Tube Drive, Reading, PA 19605, Plant 2 Casting Floor, South Mold #1 - On or about September 8, 2023, a caster operator working near molten copper was observed to be wearing a flame-resistant work coat that was damaged with tears and holes. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
