29 CFR 1926.451(e)(1): Scaffold platform more than 2 feet above or below point of access, was not provided with a ladder or direct access from another structure or other equivalent safe access during use: a) 33-49 Casselberry Way, Monroe Twp., NJ 08831. Employer did not provided a ladder and or direct access from another structure or equivalent safe access to the Alum-A-Pole pump jack scaffold platform in that the employee stepped from the platform onto a pitched snow covered roof and then to the second floor deck with no fall protection system in place exposing the employee to fall hazards. Condition occurred on or about 01/26/16.
