29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On top of the concrete block wall, fall protection was not provided to employees installing wood floor joist and roof trusses on a residential property, exposing the employees to a 9 foot 10 inch fall hazard, on or about 1/29/16.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the jobsite, the 16 foot extension ladder that was used to access the top of concrete block wall was braced against the concrete block wall and did not extend past the landing surface, exposing employees to a 9 foot 10 inch fall hazard, on or about 1/29/16.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a. At the jobsite, the 16 foot portable extension ladder employees used to gain access to the top of a concrete block wall was not secured from displacement, exposing employees to a 9 foot 10 inch fall hazard, on or about 1/29/16.
