Along the top of the exterior cinder block walls, hard hats were not worn when receiving wooden truss frames from a mobile crane, exposing employees to a struck by hazard, on or about 2/18/16.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  Along the top of the concrete block wall, eye protection was not worn when operating a pneumatic nail gun while installing wood roof trusses, exposing employees to a struck by hazard, on or about 2/18/16.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On top of the of the concrete block walls of residential property, fall protection was not provided to employees receiving wood trusses that were being flown by a mobile crane, exposing employees to a 12 foot fall hazard, on or about 2/18/16.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the jobsite, the extension ladder employees used to access the top of a concrete block wall did not extend past the landing surface, exposing the employees to a 12 foot fall hazard, on or about 2/18/16.
