29 CFR 1910.255(b)(4): All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as electronic eye safety circuit, two hand control, or similar protection:     a)  Production Floor; On or about January 16, 2016 and at times prior thereto the employer exposed employees to crush/amputation hazards in that employees were allowed to operate the A-9 Work Station press welding machine where the point of operation was not effectively guarded by use of an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    b)  Production Floor; On or about January 23, 2016 and at times prior thereto the employer exposed employees to crush/amputation hazards in that employees were allowed to operate the C-10 Work Station press welding machine where the point of operation was not effectively guarded by use of an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    c)  Production Floor; On or about January 26, 2016 and at times prior thereto the employer exposed employees to crush/amputation hazards in that employees were allowed to operate the C-5 Work Station press welding machine where the point of operation was not effectively guarded by use of an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    d)  Production Floor; On or about January 26, 2016 and at times prior thereto the employer exposed employees to crush/amputation hazards in that employees were allowed to operate the C-6 Work Station press welding machine where the point of operation was not effectively guarded by use of an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    SMART Alabama, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.255(b)(4), which was contained in OSHA inspection number 922030, citation number 1, item number 3 and was affirmed as a final order on October 29, 2013, with respect to a workplace located at 121 Shinyoung Drive, Luverne Alabama.
