29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  Westar Roofing Corporation; 4000 Coast Guard Blvd, Portsmouth, VA 23703 located at the Supply Building:  On 01-29-2016 at about 1:30 P.M. , an employee climbed an interior fixed ladder leading to the roof when the employee missed a rung and fell 15 feet resulting in a broken right leg, and shattered right ankle.  The employer did not report the in-patient hospitalization to the Occupational Safety & Health Administration until 02-03-2015 at 2:30 P.M.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
