29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) On or about 07-26-2017 and times prior thereto, located at DLA Operations Center, 8000 Jefferson Davis Hwy. Richmond, VA 23297, Condition: Employee(s) were exposed to electrical and fire hazards from a temporary electrical extension cord supplying 110 Volts to a 3-way GFCI pigtail which had a second temporary electrical extension cord that was plugged into it in a "daisy-chained" fashion supplying electrical current to portable hand tools to accomplish roofing "detail work". A large "Maxx Air Pro" metal fan was also plugged into the 3-way GFCI pigtail that was 'daisy-chained" with the other cords. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
