29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about February 17, 2016 employees who performed setup tasks on the automatic stacker were not required to: 1) Turn off and shut down the machine(s), 2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, 3) affix lockout or tagout devices to each energy isolating device, 4) prior to starting work on the machine that have been locked out, verify that isolation and deenergization of the machine or equipment have been accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 18, 2016 the in-running nip points on the ends of the stacker conveyors were not guarded to prevent employee exposure to the pinch point hazards. b) On or about February 18, 2016 the upper portion of the shear was not guarded to prevent employee exposure to pinch point and struck-by hazards. c) On or about February 18, 2016 the automatic stacker was not guarded on either loading side to prevent employee exposure to struck-by and/or pinch point hazard from the unguarded moving components. Skana Aluminum Co. was previously cited for a violation of this Occupational Safety and Health Standard 1910.212(a)(1) which was contained in OSHA inspection number 1071598, citation number 1, item number 2 and was affirmed as a final order on August 27, 2015, with respect to a workplace located at 2009 Mirro Drive, Manitowoc, WI 54220.
