Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems, Safety net systems, or Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems:     On or about May 9, 2017 employees loading aluminum slabs onto transfer carts were not protected from fall hazards over four feet when climbing on top of slabs to place spacers and/or orient slabs.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       a) On or about May 9, 2017 the rotating chuck on the 25" Monarch horizontal engine lathe was not guarded to prevent potential employee exposure to entanglement and/or flying chip hazards.        b) On or about May 9, 2017 the rotating chuck on the 2516 x 72 Monarch horizontal engine lathe was not guarded to prevent potential employee exposure to entanglement and/or flying chip hazards.        c) On or about May 9, 2017 the gap created by the rotating rollers and fixed bars on the Hot Mill table on the west ends were not guarded to prevent employee exposure to the caught-in-between hazards.

1910.22(a)(2): The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.      On or about May 9, 2017 the floor and stairs leading into the hot mill basement were not kept in a dry condition. Standing puddles of water and a leaking pump contributed to slip and/or fall hazards.
