29 CFR 1926.451(b)(3): The front edge of the platform was more than 14 inches from face of the work with no guardrail systems erected along front edge nor was a personal fall arrest system used to protect employees from falling:     a) 29 Preakness Drive, Block 17, Lot 1.15, Monroe Twp., NJ 08831. Employee was installing vinyl siding while working on and from Alum-A-Pole pump jack scaffold when the front edge of the working platform was in excess of 14 inches from the face of the work with no guardrail systems erected along front edge nor was a personal fall arrest system used to protect employees from falling exposing him to fall hazards of approximately 14 feet from the platform to the ground below.    Condition Occurred on or about 02/20/16.

29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:    a) 29 Preakness Drive, Block 17, Lot 1.15, Monroe Twp., NJ 08831. Employee was installing vinyl siding while working on and from Alum-A-Pole pump jack scaffold when platform he was working on extend greater than 18 inches with the cantilevered end not secured to prevent tipping and no guardrails were in place to block employee access to the cantilevered end exposing the employee to fall hazards of approximately 14 feet from the platform to the ground below.    Condition occurred on or about 02/20/16.

29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed affecting the scaffold's structural integrity. A Competent Person did not determine if the resulting scaffold was structurally sound after different manufactures components were mixed:    a) 29 Preakness Drive, Block 17, Lot 1.15, Monroe Twp., NJ 08831. Employee was installing vinyl siding while working on and from Alum-A-Pole pump jack scaffold with the working platform being constructed from an aluminum ladder and timber planks resting and unsecured on the scaffold supports when the scaffold had not inspected by a Competent Person to determine if the scaffold was structurally sound exposing the employee to fall hazards of approximately 14 feet from the platform to the ground below.     Condition occurred on or about 02/20/16.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, post, frames, and uprights were not erected on base plate and mud sills, or other adequate firm foundation:    a) 29 Preakness Drive, Block 17, Lot 1.15, Monroe Twp., NJ 08831. Employees were installing vinyl siding while working on and from Alum-A-Pole pump jack scaffold when the scaffold had not been erected on firm foundations in that the employees used ½ inch wafer board under each upright which were bending under the weight and sliding in the muddy conditions exposing the employees to fall hazards of approximately 14 feet from the platform to the ground below.    Condition occurred on or about 02/20/16.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a) 29 Preakness Drive, Block 17, Lot 1.15, Monroe Twp., NJ 08831. Employees were installing new vinyl siding while working on and from Alum-A-Pole pump jack scaffold when the scaffold had not been inspected for visible defects by a Competent Person before each shift and after any occurrence in that the scaffold was not erected on firm foundations, mixed components were used for one of the platforms, missing guardrails systems, platforms not secured to scaffold supports and scaffold erected more than 14 inches from the face of the work.     Condition occurred on or about 02/20/16.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired:     a) 29 Preakness Drive, Block 17, Lot 1.15, Monroe Twp., NJ 08831. Employees were using a 40 foot Werner portable aluminum ladder to access the Alum-A-Pole pump jack scaffold platform when the ladder was twisted and damaged in that it had a broken lower rung exposing the employees to fall hazards of approximately 14 feet to the ground below.    Condition occurred on or about 02/20/16.
