29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Take-up Roller Area: An employee was cleaning the take-up rollers, which had not separated, while in motion, exposing the employee to amputation and caught-in hazards. The employer developed and implemented lockout/tagout procedures but failed to utilize the procedure when cleaning the take-up rollers.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use a) Take-up Roller Area: Employees mixed sodium hydroxide for cleaning the rollers for the take-up area and there was no eyewash or drench station in the mixing area in case of accidental contact with eyes or skin.
