29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 15 Brentwood Place, Monroe Township, NJ:  Employees who were performing roofing work without an adequate fall protection system in place were exposed to fall hazards of greater than 14 feet.  Hazardous condition observed on or about 2/27/16.   Ayala construction, LLC was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 996497, citation umber 1, item number 1, and was affirmed as a final order on 1/28/2015, with respect to a workplace located at 100 Remsen Ave, New Brunswick, NJ, 08901     Ayala Construction, LLC. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 952384, citation number 1, item number 2 and was affirmed as a final order on 1/29/14, with respect to a workplace located at 600 Renaissance Dr., Pine Hill, NJ 08021.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 15 Brentwood Place, Monroe Township, NJ 08831  Portable extension ladders which were being used to access the roof did not extend at least three feet above the upper landing surface of the roof.    Hazardous condition observed on or about 2/27/16.   Ayala Construction, LLC. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 951919, citation number 1, item number 2 and was affirmed as a final order on 7/7/14, with respect to a workplace located at 900 Route 73 South, Marlton, NJ 08053.
