29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:   A) 289 George St New Brunswick, NJ 08901  Tabernacle Baptist Church  Five employees operating  pneumatic nail guns and circular saws were not wearing safety glasses.  Occurred on or about 11/01/18   The Ayala Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 951919, citation number 1, item number 1 and was affirmed as a final order on 05/14/14 with respect to a workplace located at 900 RT 73 South Marlton, NJ 08053.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 289 George St New Brunswick, NJ 08902  The employer did not ensure that each employee engaged in roofing work was protected from falling. Employees were exposed to falls of approximately 23.7ft.  Occurred on or about 11/01/18   The Ayala Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1128290, citation number 1, item number 1 and was affirmed as a final order on 06/08/16, with respect to a workplace located at 15 Brentwood Place Monroe Township, NJ 08831.  The Ayala Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 996497, citation number 1, item number 1 and was affirmed as a final order on 01/28/15, with respect to a workplace located at 100 Remsen Ave New Brunswick, NJ 08901.
