29 CFR 1904.32(b)(3): The Summary of Work-Related Injury and Illnesses (OSHA Form 300A or equivalent) was not properly certified:       (a) United Diary, Inc.: On or about March 10, 2016, and times prior, the 2015 OSHA form 300A, or equivalent, Summary of Work Related Injuries and Illness was not certified by a company executive as defined in 29 CFR 1904.32(b)(4).        The United Dairy, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.32(b)(3), which was contained in OSHA inspection number 315224162, citation number 2, item number 1 and was affirmed as a final order on October 14, 2011, with respect to a workplace located at 508 Roane Street, Charleston WV 25302.
