29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  On or about March 11, 2016, the employer did not ensure the cutter (knives) on the fill side of the Pacmac Model 9500 vertical form/fill/seal machine of the Line 6 Packing Room were guarded by one or more methods of machine guarding. A Backup Packmac Operator had reached under the Pacmac machine to pull bags when their thumb came into contact with the knives of the machine.  b)  On or about March 22, 2016, the employer did not ensure the heat sealing bar on the bag-film loading side of the Pacmac Model 9500 vertical form/fill/seal machine of the Line 6 Packing Room was guarded by one or more methods of machine guarding. Pacmac operator employees' hands and/or fingers where exposed to caught-between and burn hazards during loading of new bag-film rolls.
