29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about November 13, 2024, at the facility located at N4993 6th Drive, Westfield, Wisconsin; employees operating the #4 Pacmac (Model No.: 9500, Serial No.: 9500Z189) were exposed to potential caught in hazards from the unguarded ingoing nip point of the film bar and holding arms. b. On or about November 13, 2024, at the facility located at N4993 6th Drive, Westfield, Wisconsin; employees operating the #3 Pacmac (Model No.: 9500, Serial No.: 138) were exposed to potential caught in hazards from the unguarded ingoing nip point of the film bar and holding arms. c. On or about November 13, 2024, at the facility located at N4993 6th Drive, Westfield, Wisconsin; employees operating the #2 Pacmac (Model No.: 2015, Serial No.: 201533-0008) were exposed to potential caught in hazards from the unguarded ingoing nip point of the film bar and holding arms.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the actions identified in sections (g)(2)(i)(A)-(g)(2)(i)(D) of this standard. a. On or about November 13, 2024, at the facility located at N4993 6th Drive, Westfield, Wisconsin; the employer did not ensure that wiring for the splice bar controller for #4 Pacmac (Model No.: 9500, Serial No.: 14070033) had been guarded by an approved cabinet or other enclosure to ensure employees were not as risk from potential electrical shocks from accidental contact. b. On or about November 13, 2024, at the facility located at N4993 6th Drive, Westfield, Wisconsin; the employer did not ensure that wiring for the splice bar controller for #3 Pacmac (Model No.: 9500, Serial No.: 138) had been guarded by an approved cabinet or other enclosure to ensure employees were not as risk from potential electrical shocks from accidental contact.
