29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (a) Printing department: On or about March 29, 2016 and at times prior thereto employees were exposed to inhalation hazards from tight-fitting half-mask dual cartridge painting respirator which they used as required by the employer while conducting printing paint mixing operations and cleaning rollers. The paint included but not limited to print mix (fast solvent) and Print mix(Slow solvent) which may cause sensitization by skin contact, harmful by inhalation and irritating to eyes and skin. The employees had not been medically evaluated for respirator use.

Employees were allowed to clean rollers and perform service and/or maintenance operations on Press machine SMB 4-2 without being provided with machine specific lockout/tagout procedures. Procedures were not developed, documented and utilized for the control of potentially hazardous energy. Employees cleaned Press machine SMB 4-2 rollers while they were energized. (b) Press machines SMB 4-3: On or about March 29, 2016 and at times prior thereto the employer exposed employees to hazardous energies (electrical and pneumatic) caught-in and struck-by hazards from press machine SMB 4-3. Employees were allowed to clean rollers and perform service and/or maintenance operations on Press machine SMB 4-3 without being provided with machine specific lockout/tagout procedures. Procedures were not developed, documented and utilized for the control of potentially hazardous energy. Employees cleaned Press machine SMB 4-3 rollers while they were energized.

The employees were allowed to perform maintenance and/or servicing on Bubble Trap Machine 6-1 without isolating the machine from the energy sources to prevent accidental start-up of the machine or release of hazardous energy. (b) Tying machine 7-3: On or about March 29, 2016 and at times prior thereto the employer exposed employees to hazardous energies (including electrical and pneumatic) caught-in, struck-by and amputation hazards from Tying Machine 7-3. The employees were allowed to perform maintenance and/or servicing on Tying machine 7-3 without isolating the machine from the energy sources to prevent accidental start-up of the machine or release of hazardous energy. (c) Shirring machine 9-1: On or about March 29, 2016 and at times prior thereto the employer exposed employees to hazardous energies (including electrical and pneumatic) caught-in, struck-by and amputation hazards from Shirring machine 9-1; Clams, over-wrap fingers and shirring head. Employees were allowed to perform maintenance and/or service on Tying machine 7-3 without placing locks and tags on machinery for the purpose of controlling hazardous energy sources.
