29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):   Ballistic Melting Process/California Condor Project - Noted on 4/6/16, the employer did not develop a written hazard assessment for the ballistic gel melting process.  On 4/5/16, a flash fire occurred during the process which resulted in an employee being seriously burned.  Neither the hazards of melting gels nor safe work practices have been thoroughly evaluated.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows or floors.    Ballistic Gel Melting Area/Condor Project - Noted on 4/7/16, an orange flexible cord was used to energize a Rival 18-quart roaster pot used to melt ballistic gel.  The cord was plugged into an outlet in the  main part of the garage and ran through a doorway leading to a rear work space.

29 CFR 1910.1025(l)(1)(i): Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025:     Condor Project/Ballistic Gel Melting Process - Noted on 4/7/16, the employer did not provide the required information to employees who fire lead containing bullets and which generate lead dust and fume.  The same employees also handle spent bullets when cleaning ballistic gel molds for reuse.  Employees are potentially exposed to health hazards associated with lead dust and fume exposure.

29 CFR 1910.1200(h)(3)(iv):  The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:          Ballistic Gel Melting Area/Condor Project - Noted on 4/7/16, the employer failed to train exposed employees on the new label elements (i.e., pictograms and signal words) and the new SDS formation by 12/2/13 as required by the revised Hazard Communication Standard.  Employees use hazardous chemicals such as flammable ballistic gels (Perma-Gel and Clear Ballistic) and epoxy.
