29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  (a) Maintenance Shop: Personal Protective Equipment such as ear muffs, face shields, and welding goggles were not maintained in a sanitary and reliable condition. The muffs were stored on top of a cut off saw. The goggles were stored on top of gas welding equipment. The face shield was placed on the wall, out in the open.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  (a) Caterpiller, Backhoe Loader: A portable 2.5 LB multi-purpose dry chemical fire extinguisher was not mounted. It was laying on the equipment floor. It was designed to be mounted it in the battery box.    (b) Maintenance Warehouse: A portable 20 LB multi-purpose dry chemical fire extinguisher was not mounted. It was standing on the working room floor. It was designed to be mounted on metal brackets or in wall cabinets at lower heights with the carrying handle about 3 feet from the floor.

The work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.    (a) Maintenance Shop: A DeWalt bench grinder work rests were not adjusted to maintain a minimum of 1/8 inch to prevent the work from being jammed between the wheel and the work rest.  (b) Trail Shop: A Wissota ball bearing bench grinder work rests were not adjusted to maintain a minimum of 1/8 inch to prevent the work from being jammed between the wheel and the work rest.
