29 CFR 1917.151(h)(1): Rotating parts, such as gears and pulleys, that are located 7 feet (2.13 m) or less above working surfaces shall be guarded to prevent employee contact with moving parts. Ice Delivery System- (a) The two unguarded pulleys with a single rotating V-belt located 61 1/2 inches from the floor, exposed employees to rotating hazards.

29 CFR 1917.48(j)(3): Conveyors were operated without all overload devices, guards, and safety devices in place and operable. (a) The rotating shaft at the end of the ice auger for the ice spout was unguarded exposing employees to rotating hazards of the protruding shaft. Bornstein Seafoods Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 1917.48(j)(3), which was contained in OSHA Inspection Number 985721, Citation Number 1, Item Number 1 , and was affirmed as a final order on 02/06/2015 with respect to a work place located at, Astoria, OR 97103 Note: Abatement certification and supporting documentation are required for this item.
