29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: South Hoist- (a) The 110 volts, 20 amp. receptacle was tested with the Sure Test circuit analyzer, which indicated a reverse polarity condition exposing employees to serious injuries.

29 CFR 1910.303(b)(2): Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. South Hoist- (a) The 110 volt, GFCI 20amp circuit, was missing the outlet cover, exposing the device to damage or malfunctions from exposure to wet conditions which could result in electrical hazards for employee(s). Ice House- (b) The 110 volt, GFCI 20amp circuit, was missing the outlet cover, exposing the device to damage or malfunctions from exposure to moist conditions which could result in electrical hazards for employee(s). The Bornstein Seafoods, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 985721, citation number 2, item number 1 and was affirmed as a final order on 02/4/2015, with respect to a workplace located at Astoria, OR 97103.
