29 CFR 1910.303(b)(8)(ii): Electric equipment that depends on the natural circulation of air and convection principles for cooling of exposed surfaces were not installed so that room airflow over such surfaces is not prevented by walls or by adjacent installed equipment. For equipment designed for floor mounting, clearance between top surfaces and adjacent surfaces were not provided to dissipate rising warm air. (a) At Building 808 Heavy Equipment Shop break room, the air cooled pad mounted transformer was obstructed by furniture preventing the needed natural air circulation.

29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: (a) At Building 808 Maintenance Bay, the west roll up door cover was loose and detaching from its fastening screws. Employees were exposed to struck by hazards of the falling cover.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: (a) At Building 808 Heavy Equipment Storage area, the duplex convenience outlet used to plug-in shop equipment have broken outlet cover exposing energized copper wires. (b) At Building 808 Break room, circuit #1, panel S2480, light switch cover is cracked or damaged. The US Air Force, Nellis AFB was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(1)(ii) which was contained in OSHA inspection number 536678, citation number 1, item number 2 and was affirmed as a final order on 10/26/2012, with respect to a workplace located at Nellis AFB HVAC Shop.
