29 CFR 1910.138(a):The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s):  On or about October 11, 2023, an employee tasked with adding refrigerant to a (York) chiller located in the HVAC shop on Nellis Air Force base sustained injuries to his hands when he inadvertently made contact with the R401A refrigerant. The employer failed to access and provide appropriate personal protective equipment (chemical resistant gloves) to employees exposed to chemical hazards.

29 CFR  1910.146(c)(1):The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:  On or about October 11, 2022, employees tasked with servicing machinery such as but not limited to a York chiller (model YLA0125, serial number SKDM648470) located at building 1780 on Nellis Air Force Base were not protected from atmospheric hazards in that, the entry/exit points of the machinery were not evaluated for confined space(s). Employees who regularly crawl underneath the unit(s) when turning valves, attaching gauges, and attaching hoses could become incapacitated or their ability to self-rescue impaired if the oxygen in the air somehow became displaced or otherwise compromised.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about October 11, 2023, and at least 4 weeks prior, written procedures instructing employees on how to properly isolate, blocked, or render safe stored or residual energy, were not developed and/or used at the facility. Employees assigned to perform maintenance and repairs on equipment such as but not limited to, a York chiller (YLAA0125 serial number SKDM648470) were potentially exposed to electrical hazards. Energy sources used at the facility include electricity, thermal energy, and pneumatic energy.    The U.S. Air Force was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1597100, citation number 1, item number 01 and was affirmed as a final order on December 2, 2022, with respect to a workplace located at Building 507, Hill Air Force Base, Utah.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  Building 1780, Nellis Air Force Base. The employer failed to report the hospitalization of an employee within (24) twenty-four hours as required. On September 22, 2023, an employee in the process of servicing the facility's York chiller (YLAA0125 serial number SKDM648470) was injured when his hands inadvertently contacted the R-401A refrigerant. Hospitalized for three (3) days, the incident was not reported until October 3, 2023.  The U.S. Air Force was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1375330, citation number 01, item number 01 and was affirmed as a final order on June 5, 2019, with respect to a workplace located at Gate 14, Robins Air Force Base, Georgia.
