29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  (a) At the Fuels Building 802 mezzanine floor, the walking surface directly on top of the door where employees walk to get access to the fuels shop did not have a toe board.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  (a) At the Fuels Building 802 mezzanine floor, the orange extension used to extend power to the yellow cord for the shop had torn insulation, the outer sheathing was separating from the connector.  The US Air Force, Nellis Air Force Base was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(2)(ii), which was contained in OSHA inspection number 536678, citation number 1, item number 3b and was affirmed as a final order on 10/26/2012 with respect to a workplace located at Nellis AFB, HVAC Shop.
