29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    On August 2, 2016, employees were exposed to injuries from cuts while trimming excess PVB with a razor knife where the employees were not wearing personal protective equipment for their hands on the EPBL 1 and EPBL 2 lines.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    The employer did not verify through a written certification that the workplace was evaluated to include the date of the assessment and who performed the assessment for employees required to wear gloves while trimming excess PVB.

29 CFR 1910.1200(g)(8): The employer shall maintain in the workplace copies of the required material safety data sheets for each hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s). (Electronic access, microfiche, and other alternatives to maintaining paper copies of the material safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.)    (a) On August 18, 2016, the Safety Data Sheet for Betaseal Glass Primer was not readily available for the physicians to treat an employee exposed to the primer.
