29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On or about May 16, 2016 during the structural framing and installation of roofing material at the job site located at 1720 Springhaven Cir NE, Massillon, Ohio 44646, the employer did not ensure employees working on the roof of a single family home were protected from a possible danger of eye injury, when using an air driven nail gun.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about May 16, 2016, during the installation of roofing material at a job site located at 1720 Springhaven Cir Ne, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 9.6 feet by utilizing a fall protection system.
