29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about August 15, 2017; during the structural framing and installation of roofing material at the job site located at 6300 Cardinal Hill St NW, North Canton, Ohio 44720; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material with an air driven nail gun.     The (SKWINKLES HOME IMPROVEMENT, LLC) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1)), which was contained in OSHA inspection number 1150158 and 1192634 citation number 1, item number 1 and was affirmed as a final order on (8/11/2016 and 7/19/2017), with respect to a workplace located at 1720 Springhaven Cir NE, Massillon, Ohio 44646 and 1734 Windsor Rd NE, Massillon, Ohio 44646

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 15, 2017, during the installation of roofing material at a job site located at 6300 Cardinal Hill St NW, North Canton, Ohio 44720; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 10.5 feet by utilizing a fall protection system.     The (SKWINKLES HOME IMPROVEMENT, LLC) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 1150158 and 1192634 citation number 1, item number 2 and was affirmed as a final order on (8/11/2016 and 7/19/2017), with respect to a workplace located at 1720 Springhaven Cir NE, Massillon, Ohio 44646 and 1734 Windsor Rd NE, Massillon, Ohio 44646
