29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  On or about May 20, 2020, the employer failed to ensure that the ladder used to access an upper landing surface was extended three (3) feet above the landing surface or secured to the landing surface.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  On or about May 20, 2020, the employer failed to ensure that employees used adequate eye protection when exposed to physical injury.   The Skwinkles Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1255983, citation number 1, item number 1 and was affirmed as a final order on 10/12/2017, with respect to a workplace located at 6300 Cardinal Hill Street NW. North Canton, OH 44720.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about May 20, 2020, the employer failed to ensure that employees working above 6 feet had adequate fall protection. Employees were performing roofing work and were exposed to a fall of 10 feet without adequate fall protection.   The Skwinkles Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1255983, citation number 1, item number 2 and was affirmed as a final order on 10/12/2017, with respect to a workplace located at 6300 Cardinal Hill Street NW. North Canton, OH 44720.
