29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 601-605 Tillinghast Turn, Scotch Plains, NJ 07076:   Portable aluminum extension ladders which were being used to access the roof did not extend at least three feet above the upper landing surface of the roof.  This hazard was observed on 5/12/16.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a) 601-605 Tillinghast Turn, Scotch Plains, NJ 07076:   The Werner aluminum extension ladder which was being used to access the roof had damage, such as broken and bent rungs and rails, and had not been removed from service.    This hazard was observed on 5/12/16.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) 601-605 Tillinghast Turn, Scotch Plains, NJ 07076:   Employees who were operating pneumatic nail guns were not provided with and required to wear eye protection.  This hazard was observed on 5/12/2016.  Station Builders Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1106495, citation number 01, item number 02 and was affirmed as a final order on 04/26/2016, with respect to a workplace located at 9 Wells drive, Medford, NJ 08055.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 601-605 Tillinghast Turn, Scotch Plains, NJ 07076:  Employees who were engaged in residential roofing activities were exposed to fall hazards of up to 15 feet 8 inches and were not protected by an adequate fall protection system.  This hazard was observed on 5/12/16.  Station Builders Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1126501, citation number 02, item number 01 and was affirmed as a final order on 4/26/2016, with respect to a workplace located at 85 & 87 Arlington Ave, Lakewood, NJ 08701.  Station Builders Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1106495, citation number 01, item number 03 and was affirmed as a final order on 4/26/2016, with respect to a workplace located at 9 Wells Drive, Medford NJ.
