29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On June 1, 2016, an employee climbed out of an aerial lift onto the roof of the structure where he was not tied off or protected by fall protection, thereby exposing the employee to a fall hazard of 15 feet.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. On the June 1, 2016, the employer was using an extension cord without strain relief to energize the positive pressure system for the asbestos containment, thereby exposing employees to an electrical hazard.
