29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 43 Flannery Avenue, Lakewood NJ: Employees, wearing full body harnesses with no lanyards attached to them or the proper tie off point, were exposed to fall hazards in excess of 23 feet to the ground surface below when performing roof sheathing installation and other related activities from a residential structure's second story roof with a pitch of approximately 8 on 12.  Condition observed on or about 06/02/16.   Station Builders, Inc. was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1126501, citation number 2, item number 1 and was affirmed as a final order on 04/26/16, with respect to a workplace located at 85 & 87 Arlington Avenue, Lakewood NJ.  Station Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1106495, citation number 1, item number 3 and was affirmed as a final order on 03/10/16, with respect to a workplace located at 9 Wells Drive, Medford NJ.
