29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged directly to webbing, rope or wire rope;    a) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in proper use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work were working on the second floor top plate, approximately 9 to 33 feet above a lower level, while having their vertical lifeline hooked directly to itself. The employer's ongoing obligation is to ensure that the necessary fall protection equipment is properly implemented for the duration of the fall hazard.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in proper use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work were working on the second floor top plate, approximately 9 to 33 feet above a lower level, while having their vertical lifeline installed where the employee using the lifeline could fall in excess of six (6) feet or strike the lower level. The employer's ongoing obligation is to ensure that the necessary fall protection equipment is properly implemented for the duration of the fall hazard.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:   a) Worksite - The employer does not ensure that the top steps of stepladders are not being used as a step. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work were using the top step of a stepladder to access the interior top plate above the second floor. The employer's ongoing obligation is to ensure that the top steps of stepladders are being properly used.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work were accessing unprotected edges of the north side second floor via an extension ladder, without utilizing any means of fall protection. The employer's ongoing obligation is to ensure that the necessary fall protection equipment and/or methods are in use for the duration of the fall hazard.    b) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work accessing unprotected edges of the north side third floor via an extension ladder, approximately nine feet above the lower level, without utilizing any means of fall protection. The employer's ongoing obligation is to ensure that the necessary fall protection equipment and/or methods are in use for the duration of the fall hazard.    c) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work accessing unprotected edges of the south side second floor via an extension ladder, approximately nine feet above the lower level, without utilizing any means of fall protection. The employer's ongoing obligation is to ensure that the necessary fall protection equipment and/or methods are in use for the duration of the fall hazard.    d) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work accessing while working on the frame members of trusses, approximately between nine feet above the lower level, without utilizing any means of fall protection. The employer's ongoing obligation is to ensure that the necessary fall protection equipment and/or methods are in use for the duration of the fall hazard.    e) Worksite - The employer does not ensure that appropriate fall protection equipment or methods are in proper use. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work were working on the second floor top plate, approximately 9 to 33 feet above a lower level, without utilizing any means of fall protection. 29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).   a) Worksite - The employer does not ensure that a program is in place for a competent to perform frequent and regular worksite inspections. This violation was most recently observed at 1325 Chelsea Drive, Bethlehem, PA on June 22, 2016, when employees engaged in framing work were exposed to a variety of fall and ladder hazards, but these were not identified and corrected by a competent persons inspection. The employer's ongoing obligation is to ensure that frequent and regular inspections are being completed by a competent person.    Station Builders, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1106495, citation number 1, item number 1 and was affirmed as a final order on April 25, 2016, with respect to a workplace located at 9 Wells Drive in Medford, NJ.  Station Builders, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1117761, citation number 1, item number 1 and was affirmed as a final order on May 6, 2016.
