29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) In the production area, on or about 06-24-2016, the employer failed to ensure that appropriate procedures for the control of hazardous energy were implemented to protect employees from the hazards of accidental start of the Number 2 Winder machine or effective alternative measures were used while employees were engaged in re-winding the material to manufacture paint rollers stock after the material had break, to prevent exposure to nip-points, that could result in amputations and lacerations.

29 CFR 1910.147(d)(1): The authorized employee did not have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before the authorized or affected employee turned off: a) In the production area, on or about 07-01-2016, the employer failed to ensure that an authorized employee had appropriate knowledge of all the types of energy that needed to be controlled when performing service or maintenance on the Number 2 Winder machine, which exposed him to thermal burns with melted glue at approximately 385 degreed Fahrenheit, an uncontrolled thermal energy source that expose him to first degree burns to hands.
