The crew leader and four employees were not wearing eye protection while using pneumatic nail guns.

They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The crew leader and four employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.
