29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  JOB SITE: 2147 Asher Court, O Fallon, MO 63376, on or about 4/2/2019, workers were observed on the roof actively installing roof shingles, operating pneumatic nail guns, without any form of eye or face protection.  Workers were exposed to a struck by hazard.  MOOSE CONSTRUCTION, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number  1164061, citation number 1, item number 1, issued on 7/27/2016, and was affirmed as a final order on 8/23/2016, with respect to a workplace located at 17 Kingston Court, Saint Peters, MO 63376.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 2147 Asher Court, O Fallon, MO 63376, on or about 4/2/2019, workers were observed on top of a 6:12 pitched roof, approximately 20 feet 5 inches above ground level, actively installing roof shingles, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.  MOOSE CONSTRUCTION, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1164061, citation number 1, item number 2, issued on 7/27/2016, and was affirmed as a final order on 8/23/2016, with respect to a workplace located at 17 Kingston Court, Saint Peters, MO 63376.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)  JOB SITE: 2147 Asher Court, O Fallon, MO 63376, on or about 4/2/2019, workers were observed using a Louisville Extension Ladder, Model: FE 3216 to access onto the roof and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface.  The workers were exposed to a fall hazard.  MOOSE CONSTRUCTION, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1164061, citation number 1, item number 3, issued on 7/27/2016, and was affirmed as a final order on 8/23/2016, with respect to a workplace located at 17 Kingston Court, Saint Peters, MO 63376.
