29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  a)  At 1435 Tullahoma Dr Prattville, Alabama:  On or about July 19, 2016 and at times prior; the employer allowed his employees to use a portable skill site that had a small piece of wood wedged into the guard exposing the saws blade and exposing his employees to amputation hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 1435 Tullahoma Dr. Prattville, Alabama:  On or about July 19, 2016 and at times prior; the employer allowed his employees to walk the ceiling joists above the garage without fall protection exposing them to a fall hazard of 13 feet to the concrete floor below.  b)  At 1435 Tullahoma Dr. Prattville, Alabama:  On or about July 19, 2016 and at times prior; the employer allowed his employees to stand at a wall opening to transfer lumber to the second floor without fall protection exposing them to a fall hazard of 12 feet 6 inches to the concrete floor below.  c)  At 1435 Tullahoma Dr. Prattville, Alabama:  On or about July 19, 2016 and at times prior; the employer allowed his employees to work from a walk board without fall protection exposing them to a fall hazard of 12 feet to the wood floor below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 1435 Tullahoma Dr. Prattville, Alabama:  On or about July 19, 2016 and at times prior; the employer allowed his employees to use a job made ladder on site to gain access to the second floor and the rails of the ladder do not extend three feet above the landing area exposing employees to a fall hazard of 11 feet 7 inches to the concrete floor below.
