29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On August 8, 2016 employees who remove jams on the U-Bolt #540 line machine was not trained as an authorized lockout employee, in that during the removal of jams the employees would lift the guard, remove the jam and would not utilize lockout procedures.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) Employees who perform servicing and/or maintenance on or around U-Bolt #540 machine loading area were not required to: (1) Physically locate and operate the electrical disconnect in such a manner as to isolate machine or equipment (2) Address all energy sources including but not limited to Kinetic energy, in that the machine still moves after opening the guards. (3) Affix a lockout or tagout devices to the electrical disconnect.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On August 8, 2016 while removing a jam on the U-Bolt #540 machine guarding was inadequate in that the guards did not prevent the operators from opening them.
