29 CFR  1910.132(d)(1)(iii):  Hazard assessment and equipment selection. The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  Select PPE that properly fits each affected employee. Note: Non-mandatory Appendix B contains an example of procedures that would comply with the requirement for a hazard assessment.  a) On or about January 26, 2022, the employer's hazard assessment was inadequate in that the cloth gloves provided to the AJAX machine operators did not fit properly in that the gloves were too large.

Employers shall base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified.  a) On or about January 26, 2022, the one size fits all cotton gloves provided by the employer were being used during the unjamming of the bars from the AJAX ADC feeder rack.

29 CFR  1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  Each employee was not required to: (1) turn off and shut down the machines, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (3) affix lockout or tagout devices to each energy-isolating device, (4) verify the effectiveness of energy isolation.  a) On or about , January 26, 2022, on the AJAX Machine,  each AJAX machine operator performing servicing operations did not lock out the electrical and air disconnects when performing tasks such as unjamming the bars from the ADC feeder rack.

29 CFR  1910.219(f)(3): Gears, sprockets, and chains - Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a) On or about January 26, 2022, the AJAX ADC feeder chain and sprocket was not enclosed to prevent employees from getting caught in the unguarded chain and sprocket.
