29 CFR 1910.37(b)(2) as required by 1960.8(b): Each exit was not clearly visible and marked by a sign reading "Exit":      In the Meat department, the designated exit doors from the meat cutting and meat wrapping rooms were not clearly indicated and marked with a sign reading "exit".    The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.37(b)(2) which was contained in OSHA inspection number 967332, notice number 1, item number 1 and was affirmed as a final order on July 14, 2014 with respect to a workplace located at Defense Commissary Agency located at Lemoore Naval Air Station, California 934246.

29 CFR 1910.212(a)(3)(ii) as required by 1960.8(b): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    On August 19, 2016, in the meat department, the Hobart bone in meat cutting band saw was not adjusted to cover the entire blade except for the working portion, or point of operation.    Defense Commissary Agency was previously cited for a violation of this standard in inspection number 1073789, citation number 1, item number 1 which was affirmed as a final order on August 11, 2015, with respect to a workplace located at 1300 Oceana Blvd., Virginia Beach, VA 23456.

On or about August 30, 2016, the employer did not record a workplace injury on the OSHA 300 form in calendar year 2016.    On August 19, 2016, in the meat cutting area, a recordable injury occurred when an employee suffered a amputation of the right index finger while operating a Hobart band saw.
