29 CFR 1910.303(b)(2) as required by 1960.8(b): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about March 29, 2017, the employer did not ensure that an extension cord was not daisy chained to a relocatable power tap. In the Administrative Office area, one orange extension cord was daisy chained together with a relocatable power tap to provide power to equipment such as but not limited to computer equipment.
