Napoleon Spring Works, Inc. - Archbold, Ohio: On or about November 1, 2016, the employer did not ensure that employees who performed tasks such as, but not limited to, operating press machines and torch cutting wore eye and/or face protection. Employees were exposed to eye and face injuries from flying metal debris.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Napoleon Spring Works, Inc. - Archbold, Ohio: On or about September 7, 2016, the employer failed to ensure that the Coner machines were guarded to protect employees from the rotating part hazard of the cone and pinch point hazards created by the clamps. b. Napoleon Spring Works, Inc. - Archbold, Ohio: On or about September 7, 2016, the employer failed to ensure that that the mandrel machine was guarded to protect employees from the ingoing nip point created between the wire and mandrel and from the rotating collar on the mandrel. c. Napoleon Spring Works, Inc. - Archbold, Ohio: On or about November 1, 2016, employees operating the mandrel machines were exposed to struck-by hazards in that the mandrel is not adequately secured to the chuck. During operation, a mandrel became removed from the chuck when the nail holding it in place broke. d. Napoleon Spring Works, Inc. - Archbold, Ohio: On or about November 1, 2016, employees were exposed to pinch points and rotating part hazards in that the roll form machine was not adequately guarded. Light curtain devices did not prevent employees from accessing rotating parts and punch dies at all sides and at the end of the roll form section.
