29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection:     A)    Napoleon Spring Works, Inc., located at 111 Weires Drive, Archbold, OH 43502: On or about March 27, 2018, the employer did not provide certifications that the periodic inspections of the lockout/tagout procedures for the left mandrel winder and the right mandrel winder, located in the Spring Department, were performed. The employer did not identify the machines or equipment, the dates of the inspections, employees included in the inspections, and the person performing the inspections.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A)    Napoleon Spring Works, Inc., located at 111 Weires Drive, Archbold, OH 43502: On or about March 12, 2018, the employer did not ensure that one or more methods of adequate machine guarding were provided to protect employees from hazards such as, but not limited to, the ingoing nip point created between the mandrel/bar and the wire and from moving parts such as, but not limited to, the rotating mandrels/bars and collars with two protruding bolts on the left mandrel winder located in the Spring Department. Employees were exposed to caught-in  hazards.

29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     A)    Napoleon Spring Works, Inc., located at 111 Weires Drive, Archbold, OH 43502: On or about March 27, 2018, the employer did not ensure that employees utilized specific procedures to control hazardous energy on the left mandrel winder located in the Spring Department. Hazardous energy sources such as, but not limited to, electrical were not isolated prior to performing activities such as, but not limited to, changing mandrels/bars during set-up of the left mandrel winder. Employees performing set-up tasks in this condition were exposed to caught in hazards.      B)     Napoleon Spring Works, Inc., located at 111 Weires Drive, Archbold, OH 43502: On or about March 27, 2018, the employer did not ensure that employees utilized specific procedures to control hazardous energy on the right mandrel winder located in the Spring Department. Hazardous energy sources such as, but not limited to, electrical were not isolated prior to performing activities such as, but not limited to, changing mandrels/bars during set-up of the right mandrel winder. Employees performing set-up tasks in this condition were exposed to caught in hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    A)    Napoleon Spring Works, Inc., located at 111 Weires Drive, Archbold, OH 43502: On or about March 27, 2018, the employer did not ensure that operating employees tasked with set-up activities at or near energized parts on the left mandrel winder and the right mandrel winder were adequately trained in the application of lockout procedures. The third shift operating employees were not effectively trained on energy isolation application and procedures, were exposed to energized components of the mandrel winders.
