29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: Workplace: The employer did not ensure that the gable end frame was positively secured and stable on the forks of the powered industrial truck (PIT). On or about August 11, 2016, the gable end frame fell backward and hit the operator of the PIT during the process, and the operator sustained serious injuries from the incident.
