29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    (a) At the worksite located at 10861 S. Howell Ave., Oak Creek, WI, the employer did not ensure employees use head protection when working under elevated pieces of vinyl, thereby exposing workers to struck-by hazards.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that a cable system used to suspend pieces of vinyl was not inspected to address any deficiencies with the system.     (a) On or about October 24, 2019, located in the wash bay, the hoist system's wire rope was twisted and frayed resulting in a failure, thereby exposing employees to struck-by hazards.
