29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means:  a)  On or about August 18, 2016 at Winnebago Drive and Roosevelt Street in Fond du Lac, the employer failed to determine the exact depth of a gas line prior to cutting concrete road surface in preparation of digging operations.  An employee used a walk-behind saw to cut through nine inches of concrete and hit a one inch diameter gas service line located approximately nine and one quarter inches deep.
