29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):   Don E Parker Excavating, Inc. project site located along S. Park Ave and at the corner of E 1st St. Fond du Lac, WI: On August 23, 2018 two employees were exposed to engulfment hazards while mudding in a lateral concrete pipe to a concrete catch basin in an excavation that was eight feet wide, ten feet long by 4'-8.5" to 5'-11" deep with out benching side walls or installing a trench protection system.  Don E Parker Excavating Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.652(a)(1), which was contained in OSHA inspection number 1159955, citation number 1, item number 2 and was affirmed as a final order on July 28, 2016, with respect to a workplace located at Arndt and McKinley St., Fond du Lac, WI 54935.
