29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. Employees performing roofing work are not protected from fall hazards by the use of a fall protection system.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on October 4, 2016, when 4 employees installing roofing paper on the 6:12 pitch roof of a house at 2621 Sunrise Ridge Lane, Jacksonville, Florida, were exposed to an 18 feet fall hazard to the ground in that they were not attached to a personal fall protection system.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 997948, citation number 1, item number 1, and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 228 Melissa Ray Drive, Jacksonville, FL 32225.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 996902, citation number 1, item number 1, and was affirmed as a final order on March 24, 2015, with respect to a workplace located at 4324 Queensway Drive, Jacksonville, FL 32257.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 983753, citation number 2, item number 1, and was affirmed as a final order on September 30, 2014, with respect to a workplace located at 11645 Fallingleaf Trail, Jacksonville, FL 32258.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 959679, citation number 1, item number 1, and was affirmed as a final order on April 23, 2014, with respect to a workplace located at 96 Bayside Drive, Palm Coast, FL 32137.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 941605, citation number 1, item number 1, and was affirmed as a final order on November 6, 2013, with respect to a workplace located at 1421 Chablis Court North, Orange Park, FL 32067.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 931265, citation number 1, item number 2, and was affirmed as a final order on November 6, 2013, with respect to a workplace located at 2276 Gabriel Drive, Orange Park, FL 32073.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 447093, citation number 1, item number 2, and was affirmed as a final order on June 27, 2012, with respect to a workplace located at 12957 Everett Street, Jacksonville, FL 32225.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On October 4, 2016, an employee on the ground picking up nails along the perimeter of the house, working below the roofers was not wearing a hard hat, exposing himself to an overhead hazard.
