29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      On or about October 5, 2016, during the installation of roofing material at a job site located at 4186 Leona Dr. Rocky River, Ohio 44116; the employer did not ensure employees working on a residential roof were protected from a fall of a height of approximately 16.5 feet by utilizing a fall protection system.     (a) LG Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 986139, citation number 2, item number 1 and was affirmed as a final order on 4/21/2016, with respect to a workplace located at 19859 Alexander Rd., Walton Hills Ohio 44146.    (b) LG Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 915927, citation number 1, item number 1 and was affirmed as a final order on 8/13/2013, with respect to a workplace located at 2797 Wilks Ln., Westlake Ohio 44145.
