29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Packing line, #2 Station, on or about October 17, 2016, and at time prior to, the employer did not require employees to utilize lockout procedures while removing and replacing sprocket chain and gears during maintenance activity for equipment such as the packing line, exposing employees to caught-in and amputation hazards. Tifton Aluminum Extrusions, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.147(c)(4)(i), which was contained in OSHA inspection number 933343, citation number 1, item number 5, and was affirmed as a final order on March 18, 2014, and 1910.147(f)(3)(i), which was contained in OSHA inspection number 959403, citation number 1, item number 6, and was affirmed as a final order on July 8, 2014, with respect to a workplace located at 250 Southwell Blvd, Tifton GA 31794.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Packing Line, #2 station, on or about October, 2016, and at times prior too, the employer did not ensure an authorized employee received training in the recognition of the methods, means necessary for energy isolation, controls around machinery and equipment capable of releasing hazardous energy during servicing, and maintenance activities, exposing employees to amputation hazards. Tifton Aluminum Extrusions, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.147(c)(7)(i), which was contained in OSHA inspection number 933343, citation number 1, item number 6, and was affirmed as a final order on March 18, 2014, with respect to a workplace located at 250 Southwell Blvd, Tifton GA 31794.
